Information Regarding  BLUE Med Center & Spa Closure

and Medical Records Requests

We would like to update our March announcement regarding the closure of BLUE Med Center & Spa.  As many of you are aware, the closure occurred without much prior announcement due to circumstances completely beyond our control.  BLUE had strategically planned a new concept and update for months prior to the closure which unfortunately could not go forward.  Again, we had no control over the closure.  We had less than a week’s notice and there was simply no way for the business to continue.

We would like to provide updated information regarding your medical records.  Your options are:

  • Do nothing.  In this case, your records will be kept in a secure, limited access location for the required period.  In Georgia, this is 10 years from the date of your last visit.
  • You can request that your records be forwarded to the new business being established at the BLUE location although it is not yet known what the full scope of their practice will be.
  • You can request that your records be sent to you or your provider of choice.

If would like your records or if you would like your records transferred to another provider, this requires your written instruction and authorization.  Please email info@bluemedcenter.com and include “MEDICAL RECORDS REQUEST” in the subject.  A records release form will be emailed to you shortly after your email is received.  Please note that it can take up to 30 days for record requests to be processed from the date your request is received.  Your request must be in writing with the original returned by mail to the address provided on the form.
